Bullae of bullous pemphigoid were observed using scanning electron microscopy. In the early stage, epidermal-dermal separation was present between the basal cells and basal lamina. As the bulla developed, the basal cells showed marked intracytoplasmic damage, and some entered the bulla cavity. In the case of bullae induced by suction, no changes were found in either the basal cells or dermis.
