Two patients with squamous cell carcinoma arising in chronic perianal pyoderma are reported. The first patient was a 57-year-old man who had a 2-year history of chronic perianal pyoderma. Physical examinations revealed a squamous cell carcinoma on his hip and inguinal lymph nodes metastases. He received combined therapy consisting of chemotherapy, radiation and operation, but 8 months later, multiple metastases developed in the paraaortic lymph nodes and lung. The second patient was a 68-year-old man with a 20-year history of recurrent chronic perianal pyoderma. The lesion was resistant to all treatments, and later ulcer formation developed. The non-healing lesion was diagnosed to be squamous cell carcinoma and was managed with radiation and a wide excision. He showed no recurrence as of 3 years postoperatively.
