腹腔鏡下手術が診断に有用であった脾リンパ管腫の1 例

症例は69 歳，女性．健診で糖尿病を指摘され近医で腹部超音波検査を施行したところ，脾腫瘍を認め，当院を紹介受診した．精査にて悪性疾患の否定ができないため，腹腔鏡下脾臓摘出術を施行した．手術時間は2 時間6 分，出血量は少量で術後経過は良好であった．病理組織検査にて，脾リンパ管腫と診断された．悪性疾患との鑑別が困難な脾腫瘍に対し，腹腔鏡下脾臓摘出術は低侵襲で有用な術式と考えられた．

抄録

A 69-year-old woman was referred to our hospital following the observation of a splenic tumor during abdominal ultrasonography at a nearby clinic, after the indication of diabetes in a medical checkup. After further inspection, malignant disease could not be denied and laparoscopic splenectomy was performed. The operative time was two hours and six minutes, with a small amount of bleeding and an uneventful postoperative course. The pathological outcome was lymphangioma of the spleen. Splenectomy is performed for splenic tumors that are difficult to differentiate. However in such circumstances, laparoscopic splenectomy is considered to be a useful and less invasive method.
