Three patients presented with chronic otitis media accompanied with extensive bone destruction without evidence of cholesteatoma.
Review of literatures suggested that bacterial infection and progressive osteitis were the causes of bony erosion in the first case. Chronic granulation tissue containing cholesterine and an expanding mastoid cyst producing pressure erosion were most likely causative agents of bony destruction observed in case 2 and 3, respectively.
