Radioisotopes (RIs) produced by accelerator technologies are becoming increasingly important not only in nuclear medicine diagnostics, but also in cancer therapy. In particular, theranostics, which integrates diagnostics and therapy, is attracting global attention as a next-generation form of precision medicine. This article discusses the medical value of accelerator technologies from the perspective of radiopharmaceutical development, manufacturing, quality assurance, supply, and clinical implementation. RIs produced by accelerators become true medical value only when they are connected with radiochemistry, formulation, Good Manufacturing Practice (GMP)-based quality control, half-life-based logistics, clinical site readiness, and patient selection. Through PDRadiophama Inc. and the PeptiDream Inc.’s efforts in theranostics, macrocyclic peptide-based Drug Delivery System (DDS), Phase 0 studies, and domestic RI supply, this article highlights the importance of the value chain “from accelerators to the bedside.” At the end of this value chain, there is always a patient.
