Down syndrome is frequently associated with congenital anomalies, among which gastrointestinal disorders are relatively common. Duodenal stenosis and atresia typically present in the neonatal period; however, mild stenosis may remain asymptomatic and undetected until adolescence or adulthood.

We report a case of a 12-year-old boy with Down syndrome who was incidentally diagnosed with congenital duodenal stenosis after foreign body ingestion. He had no history of gastrointestinal disease or symptoms such as vomiting. During hospitalization for pneumonia, an abdominal radiograph incidentally revealed a foreign body. Upper gastrointestinal endoscopy demonstrated a mild, soft membranous duodenal stenosis that impaired distal passage of the foreign body. The endoscope was able to pass through the stenosis, explaining the absence of obstructive symptoms and delayed diagnosis.

This case underscores the importance of considering congenital gastrointestinal anomalies when caring for adolescents and adults with Down syndrome.
