Capillary hemangioma is a rare vascular tumor of the gastrointestinal tract, with bleeding its main presenting feature. In most reported cases of capillary hemangioma, partial resections of the small bowel were performed. Here, we report two cases of capillary hemangioma of the small intestine diagnosed by capsule endoscopy (CE) and treated by endoscopic mucosal resection (EMR) using double-balloon endoscopy (DBE).
Case 1:A 73-year-old man with iron deficiency anemia received CE. A polypoid lesion with bleeding was found in the jejunum. DBE revealed a subpedunculated polypoid lesion─showing redness, tenseness, and contact bleeding─in the jejunum, about 2 m distal to the ligament of Treitz. EMR of this lesion was performed using DBE. Microscopically, numerous dilated, irregular, blood-filled spaces were present within the mucosa and submucosa, reflecting clinical features of capillary hemangioma.
Case 2:A 74-year-old man was referred for CE due to obscure gastrointestinal bleeding requiring blood transfusion. CE revealed a spontaneously bleeding nodule in the jejunum. Again, EMR of this lesion was conducted using DBE. Histological examination of the lesion diagnosed capillary hemangioma.
Post-resection, these patients showed no gastrointestinal bleeding.
