Percutaneous transhepatic gallbladder drainage (PTGBD) is first-line treatment for acute cholecystitis. However, endoscopic nasal gallbladder drainage (ENGBD) is now performed in some cholecystitis cases because of the many risks of PTGBD. A 70-year-old woman with upper right abdominal pain visited our emergency center. We diagnosed acute cholecystitis with cholecystolithiasis and gallbladder cancer according to laboratory findings and clinical course. ENGBD was performed since this can be used to diagnose and treat acute cholecystitis. Subsequently, laparotomy cholecystectomy was performed as definitive treatment. We consider that ENGBD is an effective treatment choice, especially in the case of acute cholecystitis associated with gallbladder cancer.
