A 50-year-old female came to our office with diarrhea. The symptom improved in the fasted infusion management. A retroperitoneal mass with contrast enhancement was shown on abdominal CT. We could not make a diagnosis with gastroendoscopy, EUS, nor blood withdrawal. As confirmed diagnosis, we proceeded EUS-FNA without any complications. Biopsy specimen showed paraganglioma (PG) which might induce hypertension as a complication. We must list PG as a differential diagnosis of retroperitoneal tumor ,and measure blood or urine catecholamine level and take a MIBG imaging and PET/CT before considering operative diagnosis and histopathological diagnosis of biopsy such as EUS-FNA.
