Abstract

We experienced a very rare case of mammary gland spindle cell carcinoma. The case was a 59-year-old woman, who came to our hospital for a right mammary gland tumor. A 6.5×6.5cm tumor was palpated at the right CAE region and was diagnosed as right breast cancer T4bN1bM0 in stage IIIb, leading to extended mastectomy (Brt+Ax +Mj+Mn+Ps). Postoperative pathohistological examination revealed spindle cell carcinoma occupying the majority of the central part of the tumor, medullary carcinoma surrounding its periphery and solid-tubular carcinoma partially. The transitional zone of the former two carcinomas were also noted. These findings confirmed spindle cell carcinoma to be predominant, thus leading to diangosis as the present diseases. Silver stain revealed little argentophilic fibers at the spindle cell carcinoma region, and immunofluorescent stain revealed the carcinoma to be keratin/CEA/lactoferrin stain-negative and vimentin stain-positive. Axillary lymphnodal metastasis and the hepatometastatic lesion resected 8 months after operation were from medullary carcinoma and spindle cell carcinoma, respectively. The patient, who presented metastases to the bone, liver and lung, died about 10 months after mastectomy. The present case in reported with some additional discussion on the related literature.
