Abstract
A case of the visceral arteriovenous malformation (AVM) which ruptured idiopathically into the retroperitoneal cavity is reported.
A 53-year-old man was complaining of chest discomfort and dizziness urgently transported to the hospital under the shock condition. From physical signs, ultrasound examination and CT of the abdominal region, an intraabdominal and anterior pararenal hematoma was suspected. Emergency angiography of the visceral artery showed AVM around the pancreas head region. An emergency laparotomy was performed to control hemorrhage. The large retroperitoneal hematoma had concealed the details of the AVM where preoperative angiography was very useful. The patient was successfully operated on and was discharged from the hospital on the 105 day after the operation. Based on this experience with AVM, we emphasize the importance of preoperative angiography, even if the patient has a limited time due to shock and so on.
