The levels of protein C (PC), antithrombin III (ATIII), α2-plasmin inhibitor (α2PI), plasminogen (Plg) and von Willebrand factor antigen (vWf: Ag) were measured in sequential plasma samples from 27 patients with disseminated intravascular coagulation (DIC). At presentation, approximately 70% of the patients had abnormal values of α2PI, vWf: Ag and vWf: Ag/factor VIII activity ratio, and PC was decreased in about 60% of the patients. ATIII and Plg were reduced only in about 40% of the patients. Among these parameters, α2PI changed in parallel with the course of DIC (as judged from the behavior of FDP and fibrinogen). Other parameters remained abnormal in many patients even when the patients achieved remission. These findings indicate that the pathophysiology of DIC and its severity are heterogeneous among the patients. Recognition of these heterogeneity would be important in selecting the suitable therapeutic regimen in each DIC patient.
