We report a 74-year-old man with cerebellar hemorrhage suspected to be due to dural arteriovenous fistula (dAVF) at the craniocervical junction. He complained of sudden headache and vertigo. Head computed tomography (CT) revealed cerebellar hemorrhage, intraventricular hemorrhage, subarachnoid hemorrhage. A 3-dimensional CT angiography and left vertebral angiography showed craniocervical junction dAVF. The feeding arteries are the left C1 and C2 radicular artery, and the fistula was drained by an anterior medullary vein. The patient underwent surgical interruption of draining vein. Postoperative angiography showed complete obliteration of the fistula.
Craniocervical junction dAVF presenting with cerebellar hemorrhage is very rare. It is very important to understand spinal venous anatomy to achieve complete obliteration of the dAVF.
