We experienced 21 cases of putaminal hemorrhage diagnosed by CT scan within 3 days after the apoplectic attack. These putaminal hemorrhage were classified by location and size of the hematoma as follows: The location of hematoma
1. Anterior (A) type: The putaminal hematoma extend forward anterior limb of internal capsule.
2. Middle (M) type: The hematoma is located within putaminal region.
3. Posterior (P) type: The hematoma extend backward posterior limb of internal capsule.
4. Antero-posterior (AP) type: The hematoma extend to both anterior and posterior limb of internal capsule.
The size of hematoma
1. s type: less than 25 mm in diameter of high density area by CT scan.
2. m type: 26 mm-64 mm
3. l type: more than 65 mm
According to our results, the location of hematoma is well related to functional prognosis, on the other hand the size of hematoma is related to mortality respectively. When size of hematoma is less than 25 mm, we do not recommend surgical treatment.
We believe that this simple classification for putaminal hemorrhage is very useful when we decide surgical indication.
