A series of 18 patients with intraventricular hemorrhage (hypertension 7, aneurysm 8, Moyamoya disease 1 and arteriovenous malformation 2) diagnosed by computed tomography (CT scan) were analysed. The relations were checked and discussed between etiology, localization and forms of ventricular hemorrhage, development, evolution and severity of symptoms and the outcome.
Morphologically, the ventricular hemorrhage was divided into 4 types.
1. V4 type with hematoma in the 3 or 4 ventricles was characterized clinically by fulminant course. 2. V3 type in the 2 ventricles was characterized by grave type, subsequently evolved into cerebral herniation, Ventricular drainage is always indicated in this type. 3. V2 type in one ventricle. 4. V1 type with the ventricular CSF containing variable amounts of fresh blood but without any casting. V1 and V2 type corresponded to the so-called benign type.
Early diagnosis of ventricular hemorrhage is of great importance for the early and late outcome of the spontaneous intracranial hemorrhage. The early and longterm results are greatly influenced by not only preexistent brain damage caused by intracerebral hematoma or vasospasm but ventricular hemorrhage.
