In this paper, an operative approach for the repair of basilar artery trunk aneurysms via Trautmann's triangular space is described and the results in five patients and reported. This approach provides a wide operative field in which to handle the aneurysm forceps more easily and affords access to 1-cm past the midline.
Except for hearing loss in one patient, no patient suffered serious neurologic sequelae following basilar trunk aneurysm clipping with this approach.
