In reconstruction of resultant defects after excision of malignant soft tissue neoplasms, flap transfer is required for coverage of exposed major vessels, nerves, tendons, or bone. A perforator flap is effectively used for reconstruction of lower extremity defects because the flap is thin and provides low morbidity. In this report, a 72-year-old female presented with undifferentiated sarcoma of the knee. She was successfully treated with a pedicled anterior tibial perforator flap. In addition, a 68-year-old male presented with malignant fibrohistiocytoma of the leg. His limb was uneventfully reconstructed with a free deep inferior epigastric perforator flap.
