It was reported that a patient, sixty year old farmer, who had been suffering from a number of annular cutaneous lesions, that were initiated from his hand and continuously generalized on upper extremitates and trunk. Latent type diabetes mellitus was found by GTT and PGTT. Then, one of the anti-diabetic medicaments, Chlorpropamide was orally given to the patient, and consequently the cutaneous lesions were disappear ed. Dermatopathologically, the vasular walls showed endothelial proliferation and focal degeneration of collagen surrounded by lymphocytic infiltration including giant cells were observed in the dermis. The PAS positive and toluidine blue metachromatic (pH 4.1) substances were also recognized.
By above findings, it was considered that a diabetic condition or disturbance of carbohydrate metabolism might be connected each other with pathogenesis of necrobiosis lipoidica diabeticorum and granuloma annulare.
