Abstract
Acne fulminans which is a rare disease showing predilection to young male has recently drawn attention. We report a case of this disease successfully treated by administration of minocycline and fosfomycin for about four months. The patient was a 15-year-old male. He abruptly developed numerous fingernail-sized, reddish, flat nodules with pustules on the upper pericaordia and upper parts of the back about three months ago. The face and neck were free of lesions. The nodules ulcerated rapidly, and he was seen with complaints of fever, mild arthralgia and general malaise. Laboratory findings revealed leukocytosis, elevated ESR, decreased serum iron, hypoalbuminemia and increased α2-globulin. Staphylococcus epidermidis and streptococcus faecalis were detected in repeated cultures. Since initial treatment with corticosteroid, cephalexin and DDS resulted in exacerbation of the skin eruptions, it was discontinued, and then, combined treatments of the above antibiotics and iron preparations was reinstituted. As a result, the lesions on the chest and back soon subsided leaving demarcated, elevated scars.
