Among 485 patients with primary oral carcinoma treated in the Department of Oral Oncology, Kochi Medical School during the past 22 years, 33 patients (6.8%) who revealed second primary carcinoma of other organs were clinically investigated, and the following results were obtained. 1) Among oral carcinomas, tongue carcinomas most frequently accompanied extra oralsecond primary carcinomas (8.8%), followed by gingival carcinomas (6.5%) and other side carcinomas (5.5%). 2) As to the sites of the second primary carcinoma, esophagus, stomach and intestine were most frequently involved (in 12 patients) and the lungs were involved to almost the same degree (in 10 patients). 3) Compared to the occurrence in females (4 of 184 patients, 2.2%), many males revealed double carcinomas (29 of 301 patients, 9.6%). 4) Of the recent 44 patients who underwent gastrointestinal fiberscopy, 5 patients (11.4%) revealed an early carcinoma in the examined organs. 5) The extra-oral carcinomas (the second primary carcinomas) which were detected by gastrointestinal fiberscopy and other screening exami nationsduring treatment of the oral carcinoma and just after (one month) the treatment were almost controlled but the lately detected second primary carcinomas were frequently uncontrolled. These results indicate that in patients with oral carcinoma, extra oralsecond primary carcinoma occurs unexpectedly frequently and that screening examinations for the early detection of the second primary carcinoma is impor tantfor the benefit of oral carcinoma patients.
