A case of xanthogranulomatous cholecystitis complicated with adenomyomatosis mimicking gallbladder carcinoma

Abstract

A 65-year-old man was treated for diabetes mellitus and hypertension at the Department of Internal Medicine in our hospital. The previous US and CT finding of gallbladder indicated adenomyomatosis. However, within this one year, gallbladder wall thickness was progressed, so, gallbladder carcinoma suspected and further examination was carried out. Although xanthogranulomatous cholecystitis was strongly suspected based on imaging findings, gallbladder carcinoma could not be ruled out; therefore, we performed a resection of the gallbladder bed. We then diagnosed the patient with adenomyomatosis and xanthogranulomatous cholecystitis by intraoperative pathological examination and ruled out malignancy. Differential diagnosis of xanthogranulomatous cholecystitis from gallbladder carcinoma is important. In adenomyomatosis with wall thickening as seen in our patient, gallbladder carcinoma is suspected; however, xanthogranulomatous cholecystitis should be considered and the appropriate surgical procedure should be selected by intraoperative pathological examination.
