Abstract

The patient was an 85-year-old man. He was admitted to our hospital for basket impaction when ERCP was performed for obstructive jaundice due to common bile duct stones. At our hospital, we attempted lithotripsy with an endotriptor, but the wire in the basket fractured and the tip remained in the bile duct. Therefore, the fractured basket wire was percutaneously collected under PTCS after the fistula caused by PTBD was expanded. The calculi were crushed using electronic hydraulic lithotripsy (EHL) and removed in a transpapillary fasion. The compositional analysis of the calculi showed a composition of over 98% of calcium carbonate. We report a case in which treatment under PTCS was useful for troubleshooting basket impaction and wire fracture.
