The aim of this paper is to examine the case of Ugo-town, where the neighborhoods are promoting the dementia prevention activity and community based dementia care. The one of these activities is “wakatake genki club”, which is supported by the local office and the neighborhood who are interested in the dementia prevention. The another is “Ugotown association of dementia Caravan mate and friends”, which is formed by the neiborhood who are interested in the community-based dementia care. In both activities, the concept of dementia prevention promotes the community based dementia care network in this town. To bridging dementia prevention and care, the role of local social worker and public health nurse is important.
