    China's first HIV infection was officially reported in 1985 and by the end of 1996, there may have been up to 200,000 people affected nationwide. In 2001, this figure probably exceeded 600,000. By 2003, the predicted number of HIV cases had reached 1.5 million. At least 80,000 individuals now have full-blown AIDS. China may soon have the largest HIV-infected population in the world, possibly 6 million cases by 2005. With infection rates rising at about 30% per year, it is feared this figure might exceed 10 million by 2010. Although the Chinese government was initially slow to accept the problem, in the late 1990s definite changes began occurring. In 2003 Premier Wen Jiabao publicly shook the hand of an AIDS patient and his government promised to introduce a range of free HIV-related services. Large preventive education campaigns are now underway. Unfortunately, there will still be many obstacles in controlling the epidemic and preventing further spread of this disease. Without doubt, China faces a serious predicament in the new millennium, and one which will pose numerous challenges for preventive medicine.

    Objective: The objectives of this study are to (1) compare the BSE surveillance systems of Japan and the United States (US), and to (2) validate the US enhanced BSE Surveillance program.
    Methods: This study compares the BSE surveillance systems in Japan and the US, specifically focusing on the procedures of initial test, diagnosis and confirmation. The study further examines the validity of statistical conclusions made in the US enhanced surveillance program based on the data collected from the BSE inspection performed by the Ministry of Health, Labor and Welfare of Japan (MHLW) and the Ministry of Agriculture, Forestry and Fisheries of Japan (MAFF) between October 18, 2001 and July 31, 2004. The inspection targeted all slaughtered healthy and high risk cattle.
    Results: The US enhanced surveillance program assumes no BSE occurrence in the normal adult cattle population and thus its inspection focuses only on high risk cattle. The BSE inspection performed in Japan, however, revealed that 0.00022% of the normal adult cattle were BSE-infected using the US criteria. Assuming that the same ratio of cattle was BSE-infected in the US, the Japan finding indicates that approximately 30% of the US slaughtered normal cattle population aged 30 months and over needs to be tested to satisfy the statistical condition used by the US (i.e., 99% confidence level).
    On the other hand, in order for Japan to perform the surveillance with a 99% confidence level (the statistical condition used by US), Japan needs inspect: (1) 60,539 high risk cattle (i.e., 60% of 100,583 high risk cattle); (2) 78% of normal adult cattle aged 30 months and over (1,088,589/1,387,522) and (3) 90% of normal cattle aged 30 months or less (1,845,138/2,050,154).
    Conclusion: The US enhanced surveillance program launched in July 2004 is based on the premise that no BSE occurs in normal adult cattle population. In Japan, however, BSE cases satisfying the US criteria have been found among the normal adult cattle. This fact suggests that the US needs to consider inspections targeting the normal adult cattle in the future. At the same time, for more efficient surveillance, Japan may need to consider BSE inspections targeting the high risk cattle population with a higher confidence level and normal adult cattle with a lower confidence level.

    Objectives: Epidemiological studies have suggested that exposure to environmental and occupational electromagnetic fields (EMFs) contribute to the induction of brain tumors, leukemia, and other neoplasms. The aim of this study was to investigate the genotoxic effects of exposure to 50-Hz EMFs, and of co-exposure to cisplatin, a mutagen and carcinogen, and 50-Hz EMFs, using an in vivo newborn rat astrocyte micronucleus assay.
    Methods: Three day-old male Sprague-Dawley rats were co-exposed to 50-Hz EMFs and 1.25 or 2.5 mg/kg of cisplatin. Brain cells were dissociated into single cells and cultured for 96 hours, then stained with acridine orange and an antibody against glial fibrillary acidic protein. The frequency of micronucleated astrocytes was counted with a fluorescent microscope.
    Results: The frequency of micronuclei was not increased in rat astrocytes exposed to EMFs alone. However, the frequencies of micronuclei in co-exposure to 2.5 mg/kg cisplatin and EMFs (7.5- and 10-mT) were significantly increased, compared with those in exposure to 2.5 mg/kg cisplatin alone (sham-exposure, 0-mT EMFs) for 72 hours (p<0.01).
    Conclusion: Exposure to EMFs alone did not have a genotoxic effect but co-exposure to EMFs increased the genotoxic activity induced by cisplatin. Our findings suggest that EMFs enhance the genotoxic effects of cisplatin.

    Objective: The purpose of this study was to clarify the relationship between forearm bone mineral density (BMD), body mass index (BMI), and body composition focusing on body fat percentage (BF%) in Japanese females 18 to 40 years old.
    Methods: Subjects were 2,280 females 18–40 years old. Anthropometric measurements were taken, and a medical history was obtained by questionnaire, including age at the time of the study and age at menarche. BF% was measured by bioelectrical impedance analysis. Forearm BMD was measured using dual-energy X-ray absorptiometry (DXA). The correlations of BMD with BMI and BF% were analyzed using a structural equation model.
    Results: The standardized regression coefficients for the path from BMI to BMD and the path from BF% to BMD were 0.538 and −0.184 respectively. The squared multiple correlation of BMD was 0.146. In addition, the standardized regression coefficient for the path from BMI to BF% was 0.896.
    Conclusion: The results showed a positive correlation between BMD and BMI and an inverse correlation between BMD and BF%. At the same time, it was noted that BF% increased with BMI. This indicated that BMD is dependant on BF% in subjects who have a similar BMI. Therefore, this study concluded that it is necessary to take body composition measurements into account when examining the relationship between BMI and BMD, especially in young females.

    Objective: The INTERMAP Study is an international cooperative study on the relationship between macro- and micro-nutrient intakes and blood pressure. The present study—ancillary to INTERMAP—is to evaluate validity of the INTERMAP Tables of Food Composition in Japan (ITJ) formulated by modifying the Standard Tables of Food Composition in Japan (STJ), including factoring in changes in weight and nutrient composition of individual foods due to cooking.
    Methods: With chemical analytical values of 96 meals prepared in two university hospitals in Japan as the “gold standard”, validity of calculated values based on the ITJ was examined for six major components (energy, protein, lipid, carbohydrate, sodium, potassium) by comparison of mean values, correlation, and linear regression analysis.
    Results: Although both the ITJ-based and STJ-based calculated values for all six components were significantly higher than the analytical values, differences from the analytical values were generally less marked for the ITJ-based values than for the STJ-based values. The STJ-based values were significantly higher than the ITJ-based values for protein and potassium. Analytical values showed slightly stronger correlations with the ITJ-based calculated values (r=0.876 for total energy, r=0.789 for lipid, r=0.832 for potassium) than with the STJ-based calculated values, except for carbohydrates.
    Conclusions: The ITJ was considered to have greater validity than the STJ. To obtain more accaurate data in nutritional surveys, food composition tables in which changes in nutrient compositions due to cooking methods are taken into consideration should be used.

    Objective: To investigate the relationships between sick building syndrome and mold in newly-built dwellings.
    Methods: Symptoms of 61 residents in 18 dwellings were surveyed by standardized questionnaires. Mold sampling was done by gravity sampling using an open Petri dish. Potato dextrose agar (PDA) and dichloran-18% glycerol agar (DG-18) were used as the culture medium.
    Results: There were 6 dwellings in which at least one inhabitant complained of one or more symptoms and 12 dwellings in which none of the inhabitants complained of symptoms. There was a tendency for the dwellings with inhabitants reporting symptoms to have larger colony forming units (CFU) on PDA than those without inhabitants reporting symptoms (p=0.1), but there was no difference in DG-18 result. There was a tendency for the dwellings with inhabitants reporting symptoms to have larger CFU of Cladosporium on PDA than those without (p=0.08), but there was no difference in DG-18 result. Significantly more Ulocladium sp. was detected in the dwellings with inhabitants reporting symptoms than in those without (p=0.03). Cladosporium cladosporioides was detected in all the dwellings with inhabitants reporting symptoms and 75% of the dwellings without. Cladosporium macrocarpum and Cladosporium herbarum were detected in 33% of the dwellings with inhabitants reporting e symptoms and none of the dwellings without (p=0.1).
    Conclusion: Cladosporium was dominant in the Japanese newly-built dwellings studied, and Cladosporium and Ulocladium were probably associated with the residents’ symptoms in these newly-built dwellings.

    Objectives: The aim of this study was to survey the prevalence of fascioliasis in slaughtered cattle in order to provide breeders with feedback which may prevent cattle fascioliasis, as well as human fascioliasis.
    Methods: The results of meat inspection and the information extracted from the Discriminative Data Base on Individual Livestock in Japan were analyzed statistically in 6,224 cattle slaughtered at the Tsuyama Abattoir during the fiscal year 2002.
    Results: The age-adjusted fascioliasis and cholangitis rates of all cattle were 2.0% and 7.2%, respectively. When analyzed by cattle breed, both rates were significantly higher in Japanese native cattle than in Holstein or Jersey (Chi-square test, p<0.01). The cattle raised in Okayama Prefecture, Tochigi Prefecture and Shimane Prefecture were significantly different from cattle raised in other prefectures in the age-adjusted fascioliasis and cholangitis rates.
    Conclusion: The age-adjusted fascioliasis and cholangitis rates of all cattle can be considered as nationwide rates for slaughtered cattle. The results of the comparisons suggest that cattle fascioliasis is related to feeding with rice straw and that the measures to prevent cattle fascioliasis might differ among prefectural governments.
