  • ―物理力学的分析による座屈荷重と安定した咬合―

    Abstract : The habitual opening and closing motion of the mandible is not the same as that of the occlusal plane. When these two motions are illustrated on the sagittal and the horizontal planes, the occlusal plane has an angle to the mandibular motion in the sagittal plane, while it has an angle to the forward direction in the horizontal plane. Therefore, when we consider a vector which describes the motion in the small part of the occlusal plane, the vector can be decomposed into the upward and forward directions in the sagittal plane, and be decomposed into the forward and outward directions in the horizontal plane. As a result, this small part does not give rise to a simple compression load but a buckling load which is eccentric to the forward and outward directions. In other words, “bite” is not the compression load which pushes vertically the object with the force along the center axis of the object, but it is an action to mash and break food efficiently by the buckling load due to the force whose direction deviates from the center axis of the inhomogeneous object. Here we analyze the direction of the force of this buckling load and that of the force plane in the simplified habitual opening and closing motion, and examine them synthetically with unevenness of the occlusal surface form, the passive structure and the jaw position. In other words, we analyze “bite” scientifically from the point of view of mechanics and describe the outline how to make it useful for the dental clinic and show one clinical case.

    Abstract : The aim of this study was to evaluate in cultures of human periodontal ligament-derived fibroblastic cells the cytotoxicity of seven materials, which were used for endodontic surgery. The results we obtained confirmed that 4META/MMA-TBB resin showed most biocompatibility and photo cure resin and apatite cement have also good compatibility. Amalgam showed higher cytotoxicity rather than another materials because there was no cell growth at the area attached with amalgam directly. It is very important to understand a quality of the materials, which we have to use being suitable for the purpose.

    Abstract : We reviewed the clinical articles about the endodontics treatment in order to recognize the importance of the endodontics treatment in holistic dental therapy. We refereed two journals, the Japanese Journal of Conservative Dentistry (Japan J. Conserv. Dent.) and Journal of Japan Endodontic Association (JJEA). The ratio of clinical papers in JJEA has been increasing every year and in Japan J. Conserv. Dent. was stable level. The numbers of clinical articles in JJEA are increasing but in Japan J. Conserv. Dent. are decreasing every year. It is very important for each dentist to establish “MY ENDODONTICS” based on proper method. We have to check the articles related to clinical studies in endodontics and have a new understanding of endodontic treatment based on EBM.

    Abstract : The purpose of this study was to investigate the thermoplasticity of raw materials for formulation a new root canal filling material with a comparatively low softening temperature, suitable fluidity and setting characteristics.

     The relationships between the properties and composition of the mixture constituted by the raw guttapercha-like materials (Jelutong), bee’s wax, and Japan wax were investigated using X-ray diffractometry, differential scanning calorimetry, and viscosity measurement of the mixture.

     The transition temperature of the wax components lowered about 10°C by the addition of Jelutong. The extent of phase transition of the mixture was related to the mixing ratio between Bee’s wax and Japan wax. The viscosity of the mixture varied with the composition depending upon both the inherent viscosity of each component and the change in the transition temperature.

     It was suggested from this study that the thermoplasticity of the mixture consisting of a waxy component was related to the transition temperature, viscosity and mutual compatibility of the components.

    Abstract : The purpose of this study was to compare the physical property (radiopacity) of six root canal materials using in the photographic density of D-speed dental film.

     This study, performed in accordance with ISO 6877, used the following six root canal materials ;

     (ⅰ) Ketac-Endo, (ⅱ) AH-Plus, (ⅲ) Canals N, (ⅳ) Pulpdent root canal sealer, (ⅴ) GP sealer, (ⅵ) Obturation Gutta Sofi. Photography-Density measurements were assessed using a Fuji Prescale Densitometer FPD-305. 6 mm of Aluminum step wedge was used as a bench-mark. When tested in the manner described in Clause 6.6.2, “All materials shall show a radiopacity not less than that equivalent to 6 mm of aluminurn,” in accordance with ISO 6877. The radiopacity decreased in the following order : Canals N, GP sealer, Pulpdent root canal sealer, Obturation Gutta Soft, AH-Plus, and Ketac-Endo. The results suggest that all materials showed a radiopacity not less than that equivqlent to 6 mm of aluminum.

    Abstract : Usualy, ultra sonic is used for cleaning of files for root canl treatment. And then, files are sterilizing with gas. The changing properties of files will occurred by cleaning and sterilizing procedure. In this experiment, files were observed by SEM after ultra sonic cleaning, in several conditions. In the results, blades of stainless steel files became flat in the case of contacted with stainless steel files or nickel-titanium files intentionally, however, any changes were not observed on nickel-titanium files.

    Abstract : Referring to the concept that it is required to ensure the sterilized state after adequate root formation and to fill the root of new cavity thus formed physically in the dental and internal therapy, we dentists are bending our efforts to do so in the daily practice all the time. During these busy days, it is questioned seriously as for whether or not there have been some selfish conviction or misunderstanding. Owing to such a busy clinical practice with an uneventful, less undulating everyday’s performance, we may be apt to fall into a snare easily. In order not to fall into a clinical snare, it may be naturally acquired by performing our practice accurately. Even we do so, a snare may be laid somewhere to entrap us.

    Abstract : The starting point of holistic dental practice is Endodontics. Depending on the method used, a treatment which at first seems helpful, may not be. Even in cases where extraction seems indicated, we may be able to save the tooth. If the treatment succeeds, the tooth may be preserved for a long time. If we use our professional knowledge to the full, we may be able to preserve the tooth through one method or another.

     Never give up is my belief. Only if there is no other option should we do an extraction. Extraction should be the very last resort.

    Abstract : Drilling a tooth canal with a rotary machine has recently become popular because of its simplicity. However, accidental fracture of drill bit within the canal has also increased.

     Here we report a successful removal method of the retained piece of drill bit during previous surgery by the other dentist.

    Abstract : The fractures of crown of teeth were occurred 26~76% in traumatic dental injuries. In this case report, we present simple fracture of crown without exposed pulp and complete fracture with exposed pulp of young permanent teeth (upper incisor) and complete fracture of crown of tooth with exposed pulp of adult permanent tooth (upper incisor). The pulp capping and composite resin restoration was performed to simple fracture of crown without exposed pulp on young permanent incisor. The pulpotomy and composite resin restoration with pin was performed to complete fracture with exposed pulp on young permanent incisor. The prognosis was good for both incisors. On the other hand, pulpectomy and root canal filling with calcium hydroxide sealer was performed to adult permanent incisor with exposed pulp, and the fragment was adhered to the remained tooth with composite resin. The prognosis was good on adult permanent incisor.

    Abstract : It is a very important element that diagnostic treatment ability of a dentist is superior, but a thing, the patient that a medical care person informs a patient of a condition and an effect of the treatment that we can expect precisely judge correspondence to the patient who premised a way of thinking of “informed consent” by thought, oneself by oneself on health of oneself voluntarily, and the will relation that can seem to be decided is necessary in order to raise an effect of dentistry.

     We judged a condition, and it has been educated that we gave a diagnosis in a clinical scene by main complaint of a patient.

     However the training we understood feelings of a partner, and to hear a story seems not to have been done by conventional clinical education.

     Good communication is need indispensability a good patient in order to get medical person relation.

     We want to try to describe the basic posture that there should be we medical care person about a way of a favorable interview.
