  • ―The morphological transition of the pulp cells after cavity preparation―

      Both pulp capping and vital amputation are essential in endodontic therapy. When these therapies are appropriately performed, the injured pulp tissue forms the reparative dentin in order to protect itself. We discussed the healing process after cavity preparation in rats on the cell level through an electron microscope. Finally, we considered a little about the differences between the pulp response after indirect pulp capping and that after pulpotomy.

      We carried out the investigation to evaluate the efficacy of Hurricaine Liquid and Gel on winking motion of rabbits from January 12 to May 30, 1981, in the oral physiology school of Kanagawa Dental College. Hurricaine used in this study is based on aminobenzoic acid and widely used in USA as the local anesthetic for surface application.

      The areas of application were first shaved and then anesthetization was given. We also used three surface analgesics similar to Hurricaine, 10%, 15%, 20%, 25% solutions of aminobenzoic acid diluted with PEG, PEG 400 and PEG 4,000 diluting agents.

      This animal study revealed that the onset of the analgesic effect by Hurricaine was obtained at 10 seconds after application and the duration was from 12 to 14 minutes, which was most effective of the rest of all. As to the aminobenzoic acid preperations. any more effect was achieved by making the concentration higher than 20%. The efficacy of aminobenzoic acid was not influenced by PEG diluting agents.

      The other investigation on irritating reaction to Hurricaine and the three similar analgesics available on the market confirmed that none of them caused any side effect that should be mentioned specially.

      In the dental field, as compared with hypodermic needle injection which ensures analgesic effect, there is a tendency to take less notice of the anesthesia by surface application. However, the administration of drugs for local anesthesia by injection is accompanied by pain and often makes the patients anxious and dreaded. In this study, we evaluated the analgesic effect of Hurricaine Liquid and Gel (Beutlich Inc., USA) on pains caused by local infiltration anesthesia, scaling of the subgingival culculus, displacement of gum, and stomatitis. Hurricaine Liquid and Gel are widely used as surface analgesic which is aminobenzoic acid type preperation.

      This study included 257 cases treated at our dental office in the health center of Kyosan Factory in the period of July 9 to November 15, 1981. 130 of 257 cases were anesthetized with Hurricaine Liquid. In this group, the age of the patients ranged from 20 to 58. 127 of 257 cases were treated with Hurricaine Gel and the age range of this group was from 19 to 62.

      This study confirmed that both types Hurricaine were valid in onset and duration of analgesic effects, and the efficacy rates were more than 97% without any side effect. Judging from the result achiebed in this study, Hurricaine seems to be of wide application in cooperation with its fragrance.

      Endodontists have concern with the prognosis of treatments on total health, even though the focal infection theory had been denyed more than thirty years ago. The purpose of this article is to see why these theories were so fluent in the past and what these had influenced upon clinical endodontic treatment planning recently. In these decades British and American reports have some tendency that adovocate the relationship among endodontics, infective endocarditis, asepsis, bacteremia and antibiotics. Langdon's report on hip replacement is far beyond our common sense while McGowan stresses the circumstance of dental profession to have a balanced view with some of the case reports. These trends are seemed alike only the faint impulse from over reacted physicians or very similar to the introduction of night mares of focal infection in the past. For instance some medical staffs had awaken serious over reactions on serum hepatitis ten years ago. There should be needed serenity and constant regards upon these informations.
