This study investigated the factors, which influence the environment of the oral cavity of terminal-stage cancer patients and sought the possibilities of medical responsiveness that can be provided to these patients for these factors as well. We tried to emphasize on maintaining the moisture content of the oral cavity as a modality for oral care. The result suggested that this modality of oral care may be capable of preventing secondary mucosal diseases and increased in the alleviation of discomfort in the oral cavity.
Subjects enrolled in this trial were revealed to have spent more comfortable and improved QOL during their terminal stages with this modality of oral care. This study demonstrated that oral care provided by the dental hygienists is a viable and beneficial means in improving the QOL of these patients during palliative care.

A questionnaire survey was conducted to investigate the dent-oral conditions, the symptoms of bone and joint, and the systemic symptoms of the patients undergoing hemodialysis. The study subjects consisted of 240 patients undergoing hemodialysis. The mean age was 60.1 years old and mean duration of hemodialysis was 9 years and 4 months.
As for the dent-oral conditions, presence of the thirst which differed from a diminished salivary secretion was suggested. Twenty percent of the patients complained of some type of dental disorder. The patients complaining of symptoms associated with the bone and joint were less than 10% of all patients and 72.1% of all patients felt fatigued and exhausted.
In addition, oral examinations were performed on 26 patients of the respondents, who were capable of coming to our hospital for a checkup of their dental condition. The mean age of these patients was 56.3 years old with mean duration of hemodialysis of 5 years and 6 months. As for these patients, the DMFT index was 21.3 and the mean number of missing teeth per person was 10.5.

We herein report a case of septic shock due to a dental infection resulting from a root canal procedure performed during a house call treatment. The patient was a 67-year-old male patient suffering from diabetes mellitus, liver cirrhosis, and chronic renal dysfunction. He presented at our emergency unit complaining of swelling and severe pain in the right cheek and neck regioms. He had undergone a root canal procedure during a house call visit by his family dentist. He developed a swelling and severe pain in the cheek at night on the day of treatment and experienced generalized fatigue. Based on these symptoms, diagnosis of acute periostitis of the mandible and acute suppurative inflammation of the cheek were established.
Although the patient received antibiotics and the abscess was drained, he went into septic shock the next day. We performed additional drainage of the lesion and administered systemic antibiotics. However, he did not regain consciousness and died 46 days after admission. The cause of death was renal dysfunction accompanied by increased uric acid concentration.
Dental treatment at home is becoming increasingly popular recently. However, careless surgical treatment may result in fatal infections and complications. Therefore, it is extremely important and necessary to pay special attention to the patient's past medical history as well as their present condition.

Metastasis of hepatocellular carcinoma to the oral cavity is considered rare. A 58-year-old male patient consulted our hospital with a chief complaint of swelling of the left upper jaw. The patient had undergone treatment for hepatocellular carcinoma, and the lesion was diagnosed as metastasis of hepatocellular carcinoma through biopsy of tumor in the oral cavity. We consulted the surgical and internal medicine departments, and concluded that control of the primary lesion and improvement of the masticatory and eating disorders are possible. Based on these results, chemotherapy and radiotherapy were performed followed by excision of the tumor.

Autoimmune hepatitis (AIH) is one entity of chronic hepatitis, in which defects of the autoimmune mechanisms are considered in the development of AIH which is characterized with middle-aged female predominance, positive circulating autoantibodies and hyper γ globulinemia. Liver diseases caused by hepatitis virus, drugs and alcohol should be excluded.
We report a rare case of AIH that was diagnosed during preoperative examination for the excision of a mandibular cyst.
An 18-year old female patient was referred to our hospital due to the presence of a radiolucent lesion in the mandibular region upon radiological examination. Her preoperative serological analyses revealed high levels of serum transaminases. Interpretation of her aggregate score introduced by the International Autoimmune Hepatitis Group was 18 prior to treatment, thus she was diagnosed as AIH.
Predonisolone revealed to be very effective for the treatment of AIH. Enucleation of the mandibular cyst was performed under general anesthesia without further dysfunction of the liver.
