    Purpose: We investigated on the relation between "attitude toward the organization and duties" and "safety consciousness" with the pharmacists and clerical staffs working in the community pharmacy, and analyzed the structure of those subconscious to examine preventive measures of dispensing errors.
    Subject and Method: The questionnaire survey was performed in 41 stores in a chain pharmacy which based in the Kansai area. The questionnaire was set with 30 question items concerning "the organizational and duties" and 38 question items of "the countermeasures against dispensing error" and so forth, for pharmacist (n=180) and clerical stuffs (n=127) in non-managerial position each, and the answer was requested by five point scale. Factor analysis (FA), structure equation modeling (SEM) and multiple regression analyses (MRA) were conducted by each occupation to confirm the differences.
    Result: As a result of FA, 5 factors related to the organization and duties and 5 factors related to the safety consciousness were extracted by occupation, respectively. As a result of SEM, it became clear that "attitude toward the organization and duties" affected "safety consciousness". The influence of factors such as "office climate" and "evaluation of the organization" were large especially. As a result of multiple Regression analyses, the method of the influence of individual factors and dummy variables were differed respectively.
    Consideration: Making the good office climate, raising evaluation of the organization and examining the organization management attitude that considered differences such as the type of job, the employment style, presence of the experience which was member of the risk management committee were supported to raise safety consciousness.

    AimMethods: Medical negligence cases were selected from legal reports published between 2000 and 2005. Data that delineated negligence on the part of nurses was examined with regard to the foreseeable duties and risk avoidance activities of the nurses involved. Qualitative and inductive analyses were carried out, and the data was coded. Codes that were conceptually similar or related in meaning were grouped into separate categories. This paper describes the analysis of foreseeable duties.
    Results: Seventeen cases were judged to have occurred due to negligence on the part of nurses. Medical accidents in these cases occurred between 1991 and 1999. The duties of the nurses that dealt with foreseeable results were analyzed and assigned to one of 7 cat2egories that included neural injuries occurring due to mistaken points and/or depths of injection, etc.
    Conclusion: Findings identified minimum essentials of required nursing knowledge and nursing skills as well as appropriate behavior for safety management education. With regard to the content of nursing education programs, further discussion of these findings is deemed necessary.

    Since osteoporosis typically affects the elderly, the visibility and operability of the packages for its remedy are crucial. Currently, weekly-dose remedies for osteoporosis are sold by four pharmaceutical companies with individual package designs. Forty eight senior citizens (24 male and 24 female) who were na?ve to our purpose participated in this experiment. They were shown four different types of packages of osteoporosis drugs and answered questionnaires to evaluate usability. The presentation order of four packages was counter-balanced.
    Principal component analysis was applied to the averaged questionnaire scores resulting two principal components. The first principal component was interpreted as an axis representing the visibility of drug packages, and the second was interpreted as an axis to discriminate appearance from operability. From the scores of the two principal components of the four package designs, we clarified the characteristics of each package design.

    [purpose] The medicinal solution leak of the injection medicine from the connection part of a syringe and the infusion solution tube has a serious influence on a patient. However, the leaked medicinal solution was not discovered easily by watching for small amount and transparency.I developed the medicinal solution leak detection paper (the following, the detection paper) that can detect medicinal solution leak by this time-colored fading, and performed the clinical use.
    [method] The thin paper that had soaked the ascorbic acid and the thin paper that had soaked the povidone iodine were made, and they were dried. These were overlapped, bound, and the detection paper was made.
    [result] Developed detection paper started fading with the quantity of liquid of 0.1mL in 10 seconds, and the sensitivity of fading was good.
    [Consideration] When the medicinal solution which leaked soaks into the detection paper, a color of the povidone iodine fades.This used the reaction that a color of the povidone iodine faded by the reduction action of the ascorbic acid.I could know medicinal solution leak by this fading and am easy to have come to make discovery by the watching.
    The color of the detection paper has discolored to the experiment that soaks 4% acetic acid liquid having the oxidation action into the detection paper.From this experiment result, I think the detection paper to be have fading reaction for medicinal solution leak of clinical used injection medicine.
    The developed detection paper was possible to make handily and cheap by using a safe medicine and the material, and even clinical use was useful.
