Here we report a case with submandibular gland swelling occurred 11 years after tumor-forming pancreatitis that eventually led to the diagnosis of IgG4-related disease. Gallium scintigraphy showed abnormal uptakes in the submandibular gland, pancreas, kidney, and prostate. Imaging was useful for making a definitive diagnosis of IgG4-related disease.

A 70-year-old patient who underwent ¹³¹I radioiodine remnant ablation at our hospital after thyroid cancer had undergone total thyroidectomy and received ¹³¹ I scintillation 10 days after administration. A whole body image of ¹³¹I scintigraphy showed abnormal accumulation in the left precordial region. Although no metastasis was observed in the chest CT at the time of the first visit, the possibility of metastasis cannot be ruled out, and the abnormal accumulation disappeared in the additional SPECT image. The patient was wearing a face mask, and it was removed during the whole body scan and placed in a chest pocket. A radiological technologist who noticed the face mask removed it during SPECT imaging. When the mask was imaged with a scinti camera, an abnormal distribution thought to be ¹³¹I was observed. Since the epidemic of coronavirus infection, most people have started to wear face masks regularly. Even if the patient taking the ¹³¹I scintigraphy has the face mask removed, it is necessary to confirm the presence and location of the face mask before imaging.

A man in his 70s underwent FDG-PET 6 months after Covid-19 pneumonia was cured. The cure was confirmed with a negative polymerase chain reaction (PCR) test. At the time of PET examination, CRP was 0.06 mg / dL, WBC number was 6300 / μL, which indicates no inflammatory reaction was observed, and no obvious symptoms such as fever were observed. CT images of FDG-PET / CT showed ground glass opacities (GGO) on the right lung S6. In FDG-PET images, moderate FDG abnormal accumulation were observed in the same area, and the Standardized uptake value (SUV) max was 3.0. Many investigators reports abnormal high FDG uptake by active Covid-19 pneumonia. It should be noted that abnormal FDG accumulation by GGOs may continue to be observed after healing.
