    Eleven human cases infested with ixodid ticks since 1984 were reported. Tick species were Ixodes persulcatus (3 cases), I. tanuki (1 case), Haemaphysalis flava (3 cases), and H. longicornis (4 cases). Two out of 4 cases by the last species occurred in external ear canal. The case by I. tanuki was the first report from Japan.

    We reported the activities in the psychiatric specialized halfway-house (Setagaya Rehabilitation Center). Demonstration of the case: 41 years old female with schizophrenia. After graduation from a high school, she was working normally as an office-lady for 3 years. But, by some schizophrenic hallucinations and delusions, she gave up her jobs and entered a mental hospital 4 times until now. The reason of frequent admissions was mainly the deficit of adaptability, especially self-supportion in her social life. They are concretely, for example, selfish stop of taking tranquilizers, abnormal wasting money, and easy abandoning jobs by any slight trouble etc.
    After the entrance in this center, we prepared her for the following system. On the first step, the patient stayed at a small group room (so-called a “Ward” as a Pre-halfway-house). She was trained there about some necessary programs to improve social deficits above mentioned by personal care. On the second step, the patient moved to a personal room (so-called a “Hostel” as a literally Halfway-house) and she was trained continuously or more severely for the same purpose. After 1 year, she finally left our institution, and removed to an apartment house with her own occupational jobs.
    We think, those mentioned effective results show more expectation of psychiatic rehabilitation outside hospital in future.

    目的:地域包括支援センターの看護職が行う独居の認知症高齢者の症状の進行段階に合わせた支援内容を明らかにする.

    方法:地域包括支援センターでの経験年数5年以上の看護職10人を対象に半構造化面接を行い,得られたデータを質的記述的に分析した.

    結果:6つのカテゴリー,36のサブカテゴリー,109のコードが抽出された.看護職は認知症高齢者の【現状を把握し今後の進行状態を予測する】ことから必要な支援を選択していた.看護職は【困ったときに思い出してもらえる人になる】ために信頼関係を築き,【地域で安心して生活できる環境をつくる】ための働きかけを行っていた.さらに症状が進むと介護保険サービスを活用し【独居生活を続けるために必要なサポートにつなぐ】ことを経て【次の支援チームへのかけはしとなる】役割を担っていた.そして,これらの支援を行ううえで基盤となるものは,常に【その人らしく生きることを支える】という姿勢であった.

    考察:地域包括支援センターの看護職は認知症高齢者と早い時期から信頼関係を築く努力をし,互助から共助へと進行段階に合わせた適時適切な支援を行っていた.そして,築いた関係性を基盤として,認知症高齢者の意思決定を支援し,その意思を尊重し,その人らしい生き方を支えることを大切にしていた.たとえどの時期からの関わりであっても常にその人らしさの軸がぶれないように支援の方向性を定めていくことが重要である.

    It has by now become accepted theory that success or failure as well as the particular course of the industrialization process is strongly influenced by cultural values. But often enough the problem is simplified by stressing the need to replace “traditional” (native) values with “rational” (Western) ones, implying that only the Western type mentality is capable of becoming the carrier of the industrialization process. I take the position that within the constraints of economic and technical conditions, very different value patterns which are “traditional” to that country, can become successful bases for industrialization and modernization. This means of course that the same technological conditions can and do permit different economic behavior patterns. I establish this point by comparing both the values and the behavior patterns of Japan and the west in the pre-industrial and the industrial stage.
    As basic cultural values I take for the West the well-known Primacy of Reason and Supremacy of the Individual over the group. For Japan I take very opposite, the Primacy of Harmony and the Supremacy of the Group over the individual. I indicate how these divergent cultural values could develop and how they manifest themselves. The impact of these principles upon economic behavior prior to and during the seccessful industrialization periods is shown in some detail, using rather well-known material from economic and management history. Specifically, three aspects of economic behavior are studied, each of them logically necessary for industrialization : (a) The process of innovation and destruction ; (b) the process of competition ; (c) the goal-setting. I try to show that in each of these three aspects of economic change, there was a marked difference in approach between the West and Japan, and that this difference can clearly be reduced to the prevalence of the respective cultural values.
    While maintaining the strong persistence of traditional values during, and their positive contribution for the success of the industrialization process, I also agree that technology itself is constantly, albeit gradually, reshaping those values in both the West and Japan, and that the two are moving gradually closer together.
