Case summary: A 74-year-old female presented with the chief complaints of lack of maxillary denture retention and poor esthetic appearance in the anterior region. She had a severe vomiting reflex and facial characteristics displayed a distinct contour in the anterior region along with protruding lips. The existing prosthesis had been modified as a treatment denture in order to establish an appropriate denture margin. A new roofless denture using magnetic attachments was fabricated to improve retention.
Discussion: Because the patient had a severe vomiting reflex, an appropriate length of denture margin was established by modifying the present prosthesis before fabricating the new prosthesis. In addition, it is thought that esthetic satisfaction was achieved by deciding the arrangement position of the anterior teeth part at the chairside.
Conclusion: In this case, poor retention and esthetic appearance were improved by using magnetic attachments and establishing an appropriate denture margin.
