要旨
2000年にカプセル内視鏡検査(Capsule Endoscopy:CE)が発表されてから,CEは今まで検査が困難であった小腸に対して侵襲性少なく検査が行えるようになった.CEは総じて比較的非侵襲的な診断ツールであるが,潜在的なCE特有の有害事象が発生する可能性がある.われわれはこのツールを日常臨床で有効に活用するために,その有害事象について十分な知識を有する必要がある.CE特有の有害事象としては滞留と誤嚥がある.パテンシーカプセル(Patency Capsule:PC)による腸管開通性評価はCE滞留を回避する重要な手段であるが,PC自体にもコーティング膜の遺残や腸閉塞,さらにPC局在の誤判定に起因するCE滞留などの有害事象がある.これらの有害事象を低減するためには,CEのみならずPCによる開存性評価を行う患者を選択する際,問診による詳細な医療情報を取得する必要があると考えられる.
Abstract
Since capsule endoscopy (CE) was introduced in 2000, it has become a less invasive procedure for examining the small intestine, which was previously a difficult area to study. CE-specific adverse events include retention and aspiration. Although the evaluation of intestinal patency using a patency capsule (PC) is an important means of avoiding CE retention, the PC itself also causes adverse events such as a residual coating film, intestinal obstruction, and CE retention caused by the misjudgment of PC localization. To reduce the occurrence of these adverse events, detailed medical information should be obtained through interviews when selecting patients for patency evaluations using PC and CE.
Ⅰ はじめに
2000年にカプセル内視鏡検査(Capsule Endoscopy:CE)が発表されてから,CEは今まで検査が困難であった小腸に対して侵襲性少なく検査が行えるようになった
1).本邦において2007年からCEが実臨床に用いられるようになった.当初は原因不明の消化管出血のみを対象として,Given Imaging社(現Medotronic社)のPillCam SBとOlympus社のENDO CAPSULEが用いられ,小腸における出血性疾患がその中心であった.その後,画質や性能の向上とともに,予め腸管開通性を評価するパテンシーカプセル(Patency Capsule: PC)が登場したことにより,PillCam SB2はクローン病をはじめとする狭窄を有する疾患に対しても適用拡大がなされた.さらに2014年からは小腸カプセル内視鏡(Small Bowel Capsule Endoscopy:SBCE)だけではなく大腸カプセル内視鏡(Colon Capsule Endoscopy:CCS)も保険収載された.2021年には原因不明の消化管出血のみが保険適応となるが,長瀬産業から欧米ですでに用いられていた360°パノラマ撮影を行うCapsoCam plusも登場し日常臨床においてCEが普及してきている.
CEは総じて比較的非侵襲的な診断ツールであるが,潜在的なCE特有の有害事象(Adverse Event:AE)が発生する可能性がある.特に低侵襲であるという認識のため,AEが生じ治療介入を要する場合は医療従事者のみならず患者にとっても精神的身体的負担が多くなる.
このためわれわれはこのツールを日常臨床で有効に活用するために,その有害事象について十分な知識を有する必要がある.本稿では主にCEそのもののAEとPillCamファミリーのPCによるAEについて概説する.
Ⅱ CEによるAE
CEによる偶発症として最も留意すべきものは滞留である(Figure 1).滞留とはCEを行ったのち2週間以上経過しても腸管の狭窄などにより,腸管内に留まることである.またはCEを除去するために内視鏡的または外科的介入を実施する必要がある場合と定義されることもある
2).滞留は,CEが狭窄部に嵌頓することで腸閉塞を引き起こす可能性があるAEである.最新のレビューではCE全体の約1.4%の発生割合であると報告されており
3),0-13%の範囲で報告されている
4)~10).773人の健康なボランティアを対象とした試験ではCE滞留の症例は認められなかったが
11),再発する腹痛,膨満,嘔吐などの症状はCE滞留のリスク増加と関連していると報告されている
12)~14).最も高いCE滞留割合を有するのは,小腸閉塞症状を有する患者(10-20%)
15),16),または小腸腫瘍患者(10- 25%)
17),18)であった.さらに,クローン病(Crohn’s Disease:CD)
10),19),小腸切除歴
20),腹骨盤放射線療法,非ステロイド性抗炎症薬(NSAID)の慢性使用
21)~23)はすべてリスク増加と関連していると指摘されている.さらにCEのバッテリー駆動時間内に全小腸観察ができなかった場合も,CE滞留の可能性に関連がある
2).最近のメタアナリシスでは,小腸出血が疑われる患者,炎症性腸疾患(Inflammatory Bowel Disease:IBD)が疑われる患者,既往症のIBD患者におけるカプセル滞留率がそれぞれ2.1%,3.6%,8.2%であったことが報告されている
24).約20年間における系統的レビューとメタアナリシスではCE滞留の頻度は0.73%(1,096/86,742;95% confidence interval[CI]0.59-0.89%;289 studies)であった
25).同じメタアナリシスにおいてCE滞留の原因で最も多かったものは,対象疾患がCDであること(35.4%)であった.さらに滞留したCEに対する介入は手術が最も多く(46%)(Figure 2,3),続いて内視鏡による回収(26%),薬物療法(5%)であった.しかし滞留したCEに対しての介入がなかった報告も認められた(23%)

既知のIBDは炎症による腸管ダメージの蓄積により器質的な狭窄を生じていることがあり,CE滞留を生じやすくなる
26).Pashaらは確定診断されたCDにおけるCE滞留率は4.63%(95% CI 3.42-6.25%)であったと報告している
27).
一方でCE滞留の報告は経時的には減少している
25).これは,CE滞留に関わる危険因子に関する認識の向上が反映している可能性があるかもしれない.
滞留したCEは無症状で小腸内に数カ月間留まる場合もあれば,その後の経過中に自然排出される場合もある
5),28)~31).したがって,悪性腫瘍が疑われる場合を除き,無症状の患者に対する保存的観察がされることもある
32).CDでは,ステロイドなどの標的治療を短期間行うと,滞留したCEの20-30%が排出される可能性も指摘されているが
33),34),CE滞留が持続する場合はDevise assisted enteroscopy(DAE)による回収も検討される
35).実臨床においても適切な経過観察中に滞留したCEの24%が排出され,内視鏡による回収が必要だったものは44%であったと報告されている
36).12件研究を対象としたレビューでは,ダブルバルーン小腸鏡検査による滞留したCEの回収成功率は86.5%(95%CI[75.6-95.1])であり,手術による介入の必要性が大幅に低下したと報告している(7.2% vs. 38.5%,P=0.002)
37).一方で原疾患の治療目的やDAEによる回収が失敗した場合,CEの回収とともに外科的介入が行われる場合がある
38).しかし長期にCEが滞留していることで,数年後に腸閉塞をきたした症例報告が報告されている
39)~41).さらに腸穿孔やカプセル崩壊などの合併症もまれではあるが報告されている
42),43).したがって,CE滞留後に症状を有さない場合,緊急での回収は絶対的な必要性はなく慎重な経過観察や計画的な介入は合理的な選択肢であるが,長期にわたるCE滞留は遅発性合併症のリスクにさらされる可能性がある
41).CE回収の最適なタイミングに関するデータは不足しているが,自然排泄のほとんどのケースは通常,1-3カ月内に認められるため
44),45),3カ月経過してもCE滞留が持続しているのであれば積極的に回収を試みるべきである.
もう一つのCEの偶発症としては誤嚥が考慮される(Figure 4,電子動画 1).誤嚥の定義は,気管支に迷入したCEと定義される
46).系統的レビューとメタアナリシスでは誤嚥の頻度は0.0002%(5/23,449;95% CI 0.00-0.00%;86 studies)であり,非常にまれであった
25).しかし気管支に嵌頓すると,生命を脅かす急性呼吸困難を引き起こす可能性があり,患者の半数以上がカプセル吸引後に気管支鏡による介入を必要としたと症例報告に記載されている
46)~48).加齢や神経障害などにより嚥下障害や咽頭反射が弱い患者は,CE誤嚥のリスクが高くなると指摘されている

Ⅲ CEによるAEを防ぐための工夫
CE滞留は腸管狭窄が原因であるため,狭窄を生じうる疾患に対してCEによる検査を行う場合は予め狭窄の有無の把握や腸管開通性の評価を行うべきである.このため欧米のCEに関わるガイドラインにおいて,欧州消化器内視鏡学会European Society of Gastrointestinal Endoscopy(ESGE)ガイドラインではCDにおいて,閉塞症状や既知の狭窄が認められる場合,magnetic resonance enterography(MRE)やcomputed tomography enterography(CTE)を行うことを推奨している.これらのcross sectional imagingで判断できない場合PCを用いて開通性を評価した上でCEを行うことを推奨している
32).YadavらはPCと放射線検査の陰性的中率は同等であり,これらの検査で狭窄が否定された場合,CEは滞留せずに安全に検査ができると報告している
50).一方でOrmeciらはCEの前にCTで腸管狭窄性病変をスクリーニングしても,CEの滞留は予測できなかったとも報告している
51).さらにRozendornらはMREによってCEの滞留予測がなされてもMREはspecificity(59%),陽性適中率(40%)であり,PCによる開通性評価をすることを提案している
52).PCは小児CDにおいても,腸管開通性を評価するために有用なツールとして評価されている
53).
実際,閉塞症状や既知の狭窄がなく,小腸切除歴のないCD疑いの患者におけるCE滞留のリスクは低く,小腸出血が疑われる患者のリスクと同様であると考えられる
54)~56).したがって,CE滞留のリスクを判断するには慎重な病歴が最も大切であり,CEを施行するすべての患者におけるCE滞留を予防するためにPCによる開通性評価を行う必要はない.その上で,リスクが高い患者にはPCによる開通性評価を積極的に行うべきである
57).
誤嚥に関しては高齢者や嚥下障害を有する合併症を持つ患者において,重大な合併症につながる可能性があるためその施行自体を再検討すべきである.
Ⅳ PCによるAE
PCはパリレンでコーティングされた10%バリウムが含有されたラクトースで構成されている.カプセル周囲から時間とともに浸透してきた腸液により,ラクトースが溶け最終的にカプセルはCoating膜を残し崩壊する.このPCが崩壊せず,原型で体外排出もしくは大腸まで到達していた場合,同じ形状であるCEが小腸に滞留するリスクは極めて低い
58).多くのケースシリーズ
59)~61)とメタアナリシス
62)により,PCの腸管開通性の精度は感度97%(95% CI[93-99]),特異度83%(95% CI[65-94]),受信者曲線下面積(ROC)0.96と報告されている.また近年のシステマティックレビューにおいてPCを用いた腸管開通性評価を行った場合,CD確診例におけるCE滞留率は4.63%(95% CI [3.42-6.25])から2.88%(95% CI[1.74-4.74])へ半減している
27).
PCはCE同様この20年で刷新されており,第1世代のPCはRadio Frequency Identification(RFID)タグを有し腸液が浸透する窓が片側のみであった.この結果,崩壊するまでに100時間程度を要し,狭窄部への嵌頓が生じた場合,PCが崩壊せず侵襲性のある治療を要する腸閉塞などが生じることが報告されていた
61),63).このためカプセル両側から腸液が浸透する窓がある第2世代のPCが開発された.崩壊開始までの時間が33時間程度となり,第1世代で生じていたリスクは軽減されたと報告されている
59),60),64).欧米ではRFIDタグが含有している第2世代のPillcam patency capsuleが用いられている.これは,Agile PCの名称でも用いられており,この二つは同一製品である.
しかしAgile PCにおいてもCE滞留以外の有害事象の報告があり,主には腹痛などの症状を有するAgile PCの停滞や腸閉塞
60),65)~69),少数の早期崩壊もしくは崩壊遅延とそれに伴い生じた腸管虚血の報告がある
70)~72).第1世代,第2世代PCが有していたRFIDタグは専用のスキャナーを用いてPCが体内に存在するか否かだけを判断するものであり,腸管開通性評価を厳密には判断できない.加えてRFIDタグそのものによる腸閉塞などのリスクが考慮された.これらのことから2017年12月からRFIDタグを探索するスキャナーが発売中止となった.
一方で日本ではRFIDタグを除したPillcam Patency Capsule(PC)が2012年7月に発売された.PCはRFIDタグがないこと以外はAgile PCと性能は同じである
73).このため,PCは30-33時間以内での体外原型排出確認もしくはレントゲン,CT,トモシンセシス,腹部USなどでの大腸到達確認が,腸管開通性の確認評価となる
74)~76).
Ⅴ 本邦におけるPCのAE
PCの適切な使用法を明らかにすることを目的として,本邦で行われた大規模多施設前向き研究(the Japanese Association for Capsule Endoscopy-Oriented Appropriate Use Survey for Patency Capsule(J-POP) study)では体外原型排出と正確な局在判定が重要であり,0.51%の患者にCEの滞留を生じたが,ほとんどの患者においてCEの滞留の予防が可能であった
77).一方,症例報告ではあるが本邦のPCにおいてもCE滞留以外に幾つかのAEが報告されていた
78)~85).このため,われわれはPCに関連した有害事象を解析し,AEの背景を明らかにすることを目的としたPCの安全性に関する後ろ向き多施設研究を行った
86).
2012年6月から2014年12月までの期間内にJ-POP studyに参加した41施設でPCによる腸管開通性評価を行われた2,578人の患者のうち,61人の患者で74件のAEが認められた(2.37%).このうち,11人の患者には複数のAEを生じていた.AEはコーティング膜の遺残25件(0.97%),PCによる腸閉塞23件(0.89%),CE滞留10件(0.39%),腹痛7件(0.27%),吐き気4件(0.16%),誤嚥2件(0.08%),PC排泄時の肛門不快感1件(0.04%),消化管出血1件(0.04%),33時間以内のPC崩壊1件(0.04%)が報告された.CE滞留はすべてPC局在の誤判定により生じていた.特にPCの局在判定において腹部単純X線単独による評価は判定が極めて困難であると考えられる(Figure 5).コーティング膜の遺残は25件(0.97%)生じており,これが本研究で最も多いAEであった.88%は無症状で偶発的に発見されたものであったが(Figure 6),12%の症例はコーティングにより腸閉塞を生じていた.Saitoらは,3例のコーティング膜遺残の報告をしており,うち1例は回腸クローン病で,横行結腸閉塞に関連した穿孔性腹膜炎を発症し,大腸切除術を施行した
80).コーティング膜の遺残は,ほとんどが無症状であるが,まれに穿孔を誘発することがある.このコーティング膜は,放射線不透過性が非常に低い材料であるパリレンで作られている.このため遺残したコーティング膜は単純レントゲンでは検出困難であり,このAEの実際の発生率は,特に無症状の患者でははるかに高い可能性がある.

PCによる腸閉塞は23人の患者(0.89%)で発生し,そのうち17人(73.9%)はPC自体に損傷がなく,そのうち20人(87%)は摂取後3日以内に症状を発現していた.1人の患者(0.039%)は手術を要したが,他のすべての患者は保存的治療により回復した.PCによる腸閉塞においてKopylovらは,1.2%(20/1,615件)で症候性の腸閉塞が生じたと報告している
68).またHerreriasらは,PCによる腸閉塞患者の中にはPCを摂取する前に腹痛の自覚症状を有していたことを報告している
60).事前の画像評価が行われている場合において,PCの症候性残存のリスクは,長い(>10cm)狭窄
52),87),複数の狭窄セグメント
80),および大きな(3cm)および/または二つの狭窄前拡張の場合に特に増加すると考えられる(Figure 7)
58),65),79),80),83),85).一方で画像評価が得られていない状況であっても,本邦における多施設研究の結果からは,毎食後の腹痛を有することがPCによる腸閉塞を生じることと関連していることを明らかにした.また医師のPCの経験が1年未満であることも要因として抽出された
86).つまり医療従事者の経験が浅いことが患者の不適切な選択に関連した可能性があり,CEのみならずPCによる開存性評価のために患者を選択する際,問診による詳細な医療情報を取得する必要があると考えられる.
PC誤嚥は2人の患者(0.08%)で発生した.Takedaらは,77歳の男性がPC摂取後に呼吸困難を発症し,気管支間部に局在していたPCが軟性気管支鏡により除去したことを報告した
81).PC誤嚥はCE誤嚥と同様に非常にまれであると考えられるが,高齢患者や虚血性脳卒中の既往歴のある患者など,嚥下機能不全に陥っている患者は,この合併症を起こしやすい可能性がある

Ⅵ PCによるAEを防ぐための工夫
PCはCE滞留のリスクを最小限に抑えるために用いられる.本邦におけるデータではPCを使用して開存性を確認した後でも0.39%の頻度でCE滞留が発生したが
86),これは欧米における報告(2.88%)よりはるかに低い頻度である
27).加えて,報告された症例は全例において体内に遺残していたPCの局在判定の誤りによるものであった.このことから,正確な位置判定を行うことがPC後のCE滞留を未然に防ぐ上で極めて重要であると考えられる.臨床現場では,腹部単純X線撮影でPCの位置を評価することが多いが,正確な判定は難しく,誤判断を引き起こす要因となった可能性があるためCT,腹部超音波,MRI,トモシンセシスなどのより正確な断面イメージング手段を使用する必要がある

またPCとCEの間隔が1週間を超えると,PCの形態的変化により誤った判断が生じ,CEが保持される可能性が高くなる
50),74),88),89).また腹部腫瘤,血中ヘモグロビン濃度が10g/dL未満,PC検査時間が33時間以上であることなどがCE滞留に関連する因子としてJ-POP studyでも報告されている

一方,目視で確認できずに排便時にPCが排泄される場合もある.その際も同様に開存性評価のための腹部単純X線検査が行われるが,PCの存在は認められない.開通性評価は目視の確認が基本となるためCEを施行するべきか議論が分かれるが,このような症例にCEを施行したところ,CEの残存が起こらなかったとの報告がある
90).しかしまれではあるが,早期に溶解する可能性があり,十分な画像評価が必要である

コーティング膜の遺残は,無症状で経過することも多いが腸閉塞を生じる可能性がある.したがって,開通性評価が得られずCEを施行しなかった場合であってもDAEなどで狭窄部の精査を行い遺残の有無を確認することが望ましい.将来的には分解性のPCコーティング膜の開発が期待される.
Ⅶ おわりに
CEは治療介入を要するAEを生じうることを認識すべきである.一方で低侵襲なモダリティであるため,患者にとっても重要な検査法の一つである.このため有害事象について十分な知識を身につけ,検査の適応やAEを回避するための努力を惜しまないことが日常臨床で有効に活用するために不可欠な要素となる.本稿がその一助となるのであれば幸いである.
