A man aged 48, visited the author's outpatient clinic in July 1948 with a complaint of painful swelling in the left submaxillary regions. At that time by only manually compressing against the mandibular area, a colossal sialolith, 7.2grains in weight, was easily removed from the Wharton's duct almost naturally, and soon the symptoms were cured.
In May 1950, the same patient visited the authors again with the same complaint. This time an incision on the left sublingual area was given and again a colossal sialolith, 5.2 grams in weight, was removed. With regard to this case, the existence of chronic inflammation of the salivary gland as an etiological moment is discussed.
