転移を伴える頭蓋底(Clivus Blumenbachii)脊索腫の2例

抄録

The first case: The patient was a 36-year-old female. The abducens palsy was the first sym-ptom and also complicated with tympanitis and mastoiditis. An operation was done for possible pyramidal suppration. However the symptoms of the patient became worse after the operation. Most of the right cerebral nerves were affected and metastases to the right side of the neck, the sternum and the both lungs were observed.
The patient died twenty months after the above operation. The autopsy of this patient revealed a goose egg size chordoma at the clivus, An extensive metastases of the tumor to the right cervical, the mediastinal, the paratracheal and the paravertebral glands, the both pleuras and the both lungs were observed. The second case: A 30-year-old female complained of the abducens and facial palsy at the time of admission. Physical examination revealed a tumor at the nasopharynx. The biopsy of this tumor proved chordoma. The vast dosage of Co60 (Tele-Curie) was applied to the nasopharynx, the turkish saddle and the right supraclavicular grove and the right lung where the metastasis was observed. Eight months after the admission the patient died. Autopsy findings: Chordoma invaded the major part of the anterior base of the skull. Metastases to the both cervical glands, the left and adrenal gland, the both lungs, the liver and the both ovaries were revealed.
The authors have scrutinized the literature, in regards to the primary development of the chorda, the theories of development of chordoma, the pathology, the symptoms, the diagnosis and the prognosis of this disease.
The statistical observations were made on the 34 cases reported in Japan to datd.
