The authors have examined 15 patients with sudden deafness and further progression of hearing loss after the initial examination. Eleven of the 15 patients (group A) showed the greatest degree of hearing loss during the first 8 days after the onset of deafness. The remaining 4 patients (group B) had the poorest hearing 1 month to 4 years after the sudden onset of hearing loss.
Sudden hearing loss mainly of high frequencies suggests that the sensory disturbance starts in the basal turn and then progresses towards the apical turn of the cochlea. Hearing improved considerably in the patients of group A, but not at all in those of group B. There may be a definite difference in the pathogenesis of deafness between group A and group B.
