Sebaceous carcinoma arising in the oral cavity is extremely rare. We report the case of a 76-year-old woman with delayed cervical lymph node metastases from sebaceous carcinoma of the soft palate, developing 4 and 9 years postoperatively. The primary tumor was 20 mm in diameter. On the basis of the imaging and biopsy findings, a malignant tumor of the soft palate (cT2N0M0) was diagnosed, and the tumor was excised. Delayed cervical lymph node metastasis was detected on the right side 4 years 11 months postoperatively and on the left side 9 years 8 months postoperatively. Dissection was performed on the right and left sides. The postoperative course was uneventful, and there was no evidence of recurrence or metastases 13 years after the first resection.
