多発性気管・気管支憩室を伴った Tracheobronchomegaly の一例

A 62-year-old man complained of a cervical cystic mass which increased in size by Valsalva's maneuver and cough. He had a long history of recurrent pneumonia and chronic productive cough. Chest X-ray film revealed a widened trachea and cervical cysts. Tomogram, bronchogram and CT showed that the cysts were tracheoceles arising from the widened trachea. Many small diverticula were also demonstrated around the dilated airway by bronchography. These findings were thought to be multiple diverticula of various sizes associated with tracheobronchomegaly. Bronchoscopy showed enlargement of the trachea and both main bronchi, but collapse of the posterior membraneous wall on expiration as reported by some authors was not found. Pulmonary function studies demonstrated normal values except for decrease in V25. Because no manifestation of respiratory tract infection was found, the patient was untreated. Careful follow up has been performed periodically.
