Here, we report the case of opioid dependence in a 47-year-old man with intractable chest pain. The patient had been inappropriately administered high-dose opioids (equivalent to 600 mg of oral morphine) for eight years. After admission, opioid dosage was gradually tapered, and pain was managed using alternative analgesics. The patient's mental health symptoms were treated with antiepileptic and antipsychotic medications. After 42 months, opioids were discontinued. This case highlights the importance of careful opioid management and challenges associated with opioid withdrawal in dependent patients.
