An 11-year-old boy was admitted to the authors' hospital, because of abdominal pain and vomitting for one hour. He had no previous history of abdominal disorder. Findings of laboratory data and plain x-ray films of the abdomen were within normal limits. At eight hours later, plain x-ray of the abdomen showed dilated loops of small intestine with fluid levels. Roentgen studies with gastrografin demonstrated jejunal distension. And two hours later, gastrografin moved into the normal ileum. Severe abdominal colicky pain continued, Internal hernia with strangulation was suspected, and laparotomy was carried out. The 40 cm. of ileum had herniated through a hole in the greater omentum near its free edge. The hole as a ring of 2 cm. in diameter was cut. The postoperative course was uneventful. To the authors' knowlege, this is the first case report of pediatric transomental herna in the Japanese literature.
