Introduction. Mucous gland adenoma is an extremely rare exophytic tumor of the tracheobronchial seromucinous glands and ducts. Here we report a surgically treated case of mucous gland adenoma with some discussion on the histogenesis. Case. A 63-year-old man presented to another clinic with cough, fever, and bloody sputum. He was referred to our department with a diagnosis of atelectasis of the left lower lobe of the lung. Atelectasis of the entire lower lobe and a polypoid lesion in the lumen of the bronchus of the left lower lobe were seen on chest computed tomography (CT). Bronchofiberscopic findings included a pedunculated polyp in the lower bronchus. Left lower lobectomy was performed. The pathologic diagnosis was bronchial mucous gland adenoma, and the Ki-67 labeling index was 23%. The patient's postoperative course was generally favorable. Conclusion. In the present case, complete resection was done. It is difficult to diagnose mucous gland adenoma preoperatively. Therefore, it is necessary to keep in mind this disease as one of the lesions protruding into the central bronchial lumen.
