From transient circulatory assistance to total heart replacement. Currently there are many types of cardiac prosthesis available. They are transient circulatory assist devices, interim circulatory assist devices and permanent cardiac prostheses. Left ventricular assist, biventricular assist and total replacement devices are also considered necessary. Because there are various types of prostheses with different clinical needs and different criteria for development, it is very difficult to make many different prostheses. However, a multipurpose universal cardiac prosthesis applicable to transient and to permanent cases must be developed. Due to this need, a pusher plate type prosthesis adaptable to various types of actuation modes, with only minor modifications required, was developed by the Cleveland Clinic group. A transient extracorporeal left ventricular assist device utilizing a universal plastic pump was evaluated in 2 clinical cases. A permanent, implantable left ventricular assist system utilizing a universal titanium pump was recently implanted in a calf for 40 days and demonstrated the feasibility of this approach.
