We report our investigations determining the cause of recurrence (2 cases) or non-disappearance (7 cases) in 59 cases of early gastric carcinoma treated curatively, without surgery, using the Nd-YAG laser endoscopically since 1983.
The laser was used in the 50W 0.5 pulse mode in a non-contact technique. All cases (except 2) have been followed up for more than 6 months. The 2 cases of recurrence were found 2 years after Nd-YAG therapy.
We conclude that site accessability, endoscopist experience and correct determination of extent and depth of the carcinoma are the factors responsible for the curative failure by the Nd-YAG in these 9 cases. More suitable laser probes including those for contact techniques will be helpful, as will improved operator skills.
