Abstract
Out of the total 1, 400 cases of gastrectomy performed in the Second Department of Surgery, Tokyo Uniuersity School of Medicine covering a period of 9 years from 1947 to 1956, macroscopical cases of gastric polyp were 20 and the cases of gastro-polypous cancer were 24. Stated in the followings are the findings of macroscopical and histopathological investigations made on them :
1. Sexual difference : The rate of gastric polyp between male and female was 10 : 7, while that of polypous cancer was 18 : 6.
2. Age : The age gpoup of sixties showed the highest incidence rate of both gastric polyp and polypous cancer.
3. Number : Mean of the number of polyps and cancers per case was 2.1 and 1.3 respectively.
4. Size : Gastric polyp was mostly in the size of bean, while that of cancer was mostly in the size of pigion's egg.
5. Form : In both gastric polyp and polypous cancer, they were mostly spherical.
6. Stem : In both gastric polyp and polypous cancer, nearly half of them possessed stems.
7. Etat mamelonné : Contrary to our expectation, the rate of its complication was low.
8: Histopathological characteristics : a. Elevation of the stroma was observed in 100% of both gastric polyp and polypous cancer.
b. As phenomena frequently observed, there were adenoma, proliferation of superficial epithelium, delation of capillaries, edema, cellular infiltration, encystation, elongation of faveola etc.
c. As phenomena contrary to our expectation, there were confirmed the defect of covering epithelium and the accompanying cellular infiltration.
d. Histological picture of the polypous cancer was adenocarcinoma in all cases examined.
9. The rate of gastric polyp changing into cancer was calculated to be 58.6%. Accordingly, application of gastrectomy in an early stage is considered indispensable whenever polyp is discovered.
