Simply stated the Proximal-Distal Theory of the Pathogenesis of Focal Cerebral Ischemia is : Focal Cerebral Ischemia is caused by a; Distal defect in flow or a Proximal-Distal Defect in flow and NOT by a solitary proximal defect in flow.
This concept is of primary importance in the diagnosis and treatment of active occlusive cerebrovascular disease; lack of appreciation of this concept accounts for errors in diagnosis, ineffective treatment and unwise surgical procedures. In presenting evidence to support this hypothesis I will :
