We describe relapse of hepatitis C virus (HCV) in a 43-year-old Pakistani man with chronic hepatitis C at 4 weeks after the cessation of ledipasvir/sofosbuvir therapy. Although HCV in this patient had been typed as serogroup 1, sequence analyses of the 5'UTR-core and NS5B regions revealed that he was infected with genotype 3a HCV and had a mixture of S282T (ACT) and wild-type (AGT) in the NS5B region at relapse. Of note, however, S282T was no longer detectable at 7 weeks post-treatment. This study indicates that some of serogroup-1 hepatitis C patients are infected even with HCV of genotype 3a, and that HCV genotyping should be considered for foreign hepatitis C patients before treatment with direct-acting antiviral agents in Japan.
