Background: Clinical results and its problems of arthroscopic rotator cuff repair (ARCR) for 70 years old or over were reported.
Methods: There were 57 patients and 58 shoulders, average age was 75.9 years and average follow up period was 15.8 months in this study. Preoperative physical complications, pre- and post range of motions of the shoulder, pre- and post JOA score were studied. Statistical assessment used paired t-test. Significant level was set that p value was less than 0.05.
Results: 38 patients had physical complications at preoperative time. Those were 29 cases of hypertension (50.9%), which was the most common, 14 had hyperlipemia, 10 had DM, 8 had arrhythmia and brain infarction. 8 were miscellaneous. There were 21 patients who had 2 or more complications and 23 patients undergoing anti-coagulation therapy. Following control of these complications, ARCR was performed. Elevation was improved form 104 ± 38.9 degrees to 136.9 ± 23 degrees, external rotation from 40.3 ± 23.6 to 51.3 ± 15.9 degrees, internal rotation from L2 ± 3.1 to T11.5 ± 2.6. Pain score was improved from 10.1 ± 4.1 points to 26.6 ± 4.9 points, JOA score from 58.4 ± 10.8 points to 90.4 ± 10.2 points. All these items were statistically improved at postoperative state. There were no surgical complications related to ARCR itself.
Conclusion: Control of general conditions is essential for patients of 70 years old or more. Excellent clinical results of ARCR for these patients would be obtained according to these managements.
