In most of the cases with a frozen shoulder or post-operative contracture, conservative treatments are useful and effective. But, a few cases can not respond to these conservative treatments. So, gentle manipulation or open manipulation is sometimes carried out on the unresponded cases.
There are some disadvantages (nerve injury, fracture, muscular damage etc. ) to gentle manipulation and open manipulation. In this study, we carried out gentle manipulation following arthroscopic synovectomy, and evaluated the results and complications. From September 1989 to June 1991, we treated 11 cases (9 cases: frozen shoulder,2 cases: post -operative contracture) using gentle manipulation following an arthroscopic synovectomy.
All of the cases with frozen shoulder responded well to gentle manipulation follow ing an arthroscopic synovectomy, but the two cases with post-operative contracture did not. In the cases with frozen shoulder, the pain was reduced and the shoulder function improved from 53(29-77) to 84(78-89) in the JOA score.
Gentle manipulati o n following an arthroscopic synovectomy is useful for the treatment of an advasced frozen shoulder.
