外傷性肩関節前方不安定症に対するCaspari法の短期成績

[Purpose] Since 1992, we have performed Caspari's technique for traumatic anterior shoulder instability. The purpose of this study is to evaluate the short term results and analyse the factors related to a poor result.
[Patients and methods] Twenty shoulders of 18 patients (14 males and 4 females) with traumatic anterior shoulder instability were treated with Caspari's technique. The average age at operation was 24.7years (range 16-40 years), and the mean postoperative follow-up period was 14.5 months (6-25 months). Two patients had played a collision sport preoperatively. Seventeen shoulders revealed a recurrent dislocation, and one was an initial one. Our indications for an arthroscopic Bankart repair were: 1)Instability was caused by trauma,2) There was no multidirectional instability,3) There was no bone defect of the glenoid.
[Results] According to Rowe's evaluation score,14 shoulders were excellent,2 good, and 4 poor. All the patients with a poor result had postoperative redislocations with a recurrence rate of 20%. Both patients who had played collision sports preoperatively had redislocations. The mean limitations of the range of motion was 11 degrees in external rotation at the side, and 9 degress in external rotation at 90 degrees abduction. Transient suprascapular nerve palsy occurred in two shoulders postoperatively.
[Conclusion] The recurrence rate of a dislocation after Caspari's technique was much higher than the open method. Collision sports were a risk factor to redislocate.
