To clarify the impact of advice from professionals in community care meetings and coordination by social workers on care management quality, this study analyzed the advisors and case providers who participated in the Care Plan Assessment Committee meetings, the coordinators, and the cases that were evaluated by monitoring. Results suggest that the following three factors are essential for care management that contributes to improving quality of life. First, advice from professionals should not be biased toward a medical perspective but should be elicited with attention to the perspective of daily life. Second, advice should be elicited from the perspectives of the International Classification of Functioning, Disability and Health and social work. Third, the organization of issues should be encouraged to achieve care management goals.
