Objective: We report a case of three overlapping stents placement for a treatment of blister-like middle cerebral artery aneurysm after radiotherapy.
Case Presentation: The patient was a 48-year-old female. She underwent cyberKnife treatment for residual tumor after resection of pilocytic astrocytoma. Two years after the radiotherapy, the patient was admitted due to subarachnoid hemorrhage, and a blister-like aneurysm of the dilated left middle cerebral artery was observed. The blister-like aneurysm was only 2 mm; hence, we decided not to operate immediately. A few days after admission, on angiography, the blister-like aneurysm was remarkably enlarged; therefore, we performed coil embolization, on day 10. Despite the treatment, the aneurysm ruptured again on day 22. We treated this aneurysm by stent-assisted coil embolization; however, the aneurysmal regrowth was not prevented. Thus, we placed two additional stents inside the first stent. As a result, the aneurysm was cured.
Conclusion: Placement of overlapping stents can be an option to treat blister-like aneurysms.
